Talent Release & Contract

1) I, (______________________________) represent (__________) and all individuals included in any and all videos and audio recordings that have been submitted by myself or another active or inactive member of our organization to Michael Moody, his agents and employees, and any and all individuals included in any and all videos, films, photographs and/or recordings have agreed to let me represent them and sign this contract to legally release any and all personal rights to these examples of any and all individuals included in any and all submitted videos, films, photographs and/or recordings.

2) I, (_______________________________) do hereby state that any and all individuals under the age of 18 that are included in any and all videos and audio recordings that have been submitted by myself or another active or inactive member of our organization to Michael Moody, has had their parent and/or guardian directly give me permission to represent them and sign this contract to legally release any and all personal rights to these examples of their child and/or foster child in any and all submitted videos, films, photographs and/or recordings that have been submitted to Michael Moody.

3) I, ( ______________________________) represent (___________) and do hereby give permission and grant all rights to Michael Moody, his agents, employees, assigns, clients, and/or distributors to advertise, include in video productions and release to the general public or any other party in any other country in the world, any and all videos and audio recordings that may be included in during any one of their productions and I hereby release any and all personal rights to these examples of myself and the other individuals represented in any and all videos and audio recordings , however all of the above ONLY in those recordings , that have been submitted by myself or another active or inactive member of our organization to Michael Moody, his agents and employees.

3)  I, (                ), do further state that I am over eighteen (18) years of age.

4)          I, (                ), do further understand that should either party hereto or any heir, personal or representative, successor or assignee of either party hereto, resort to litigation to enforce this agreement the party or parties prevailing in such litigation shall be entitled, in addition to such other relief as any be granted, to recover its or his reasonable attorney’s fees and costs in such litigation from the party or parties whom enforcement was sought. 


This document may be faxed or photocopied and sent to a third party for our protection.  In case the original document is lost, stolen, confiscated by any person for any reason, I hereby state that any copy or facsimile shall then serve as an original and as evidence of my agreement herein stated.
