What does the "Address/License#/Restrictions/Etc." on the order form mean?

its the type of license you want, and your address, find someones REAL license and use the same sorta thing, but obviously don't put down your own address.

Sorry bout bumping this but Im very close to buying one of these Id's Ive filled it all out and im going to send the money tomoro buy going to one of these western union things. But the email I got sed the person is from egypt? therefore how does team nowhere guy know him?
Would really like to know, and Nicco you seen on eof these before? any good?

Make your own.. I did.. works a charm.. my friend has a laminating machine so looks reall 100% ill take a pic.

A guy in my town broke into Autopac and stole the machines that make the id's haha. He charges 125 bucks for a REAL fake id. Its not sloppy at all and works everytime...its pretty awesome
