My thought: Some kind of thing over his actual arm. The coin he pulls outta his arm isnt actually a coin, but is magnetic. When he swallows it, thats gone. He has a magnet in his hand that is pulling the "coin" through his "arm" to the cut.

this guy is my favorite magician for just that reason
my theory he never swallowed the sighned coin, he had a quatar already in his mouth, totally had a fake skin around his arm and had it moving around with a magnet, either one in his hand (palming it) or one in his wrist band, also when he was rolling back his sleves is when he put the quartor in the fake arm skin
